Please read the following instructions carefully before filling the online form:

1. All the questions are in English in this online form. If you want to submit your application in Hindi, then please scroll down to see the option.
2. It will take you 15-20 mins to fill the form. We request you to fill the form without any disturbance when you have enough time in hand.
3. You must fill the form in one go. Do not switch to any other app and do not press refresh or back button.
4. Read the sample questions below to prepare yourself before filling the online form:
   • What were your key achievements as an athlete?
   • How many years of coaching experience do you have?
   • How many athletes are you coaching currently and what is their level?
   • List key achievements of your athletes.
   • Why do you want to join CEP? Name up to 5 key areas you would like to learn about during CEP

You can type your answers in a word document and email to us or you can print the form and write your answers with a pen and scan the form and email it to us. The two forms are given below:
